CarePartners is embarking on a new program in partnership with the Ontario Health atHome Central East (Durham Region) and we are looking for community-based support workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

In this exciting staffing model, HISH CSWs will have consistent hours of work, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.
The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends:
Two-week Rotation | Mon/Tues/Sat/Sun 0800 - 2000 hrs | Wed/Thurs/Fri 0800 - 2000 hrs |
This is an equivalent of seventy-seven (77) hours per bi-weekly pay period!

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
