Overview

As a progressive and creative organization, CarePartners is proud to be a leader in developing innovative programs. One of our spotlight programs is the eHomeCare Program. This is an innovative provincial program that is unique to home care and includes oversight of the patient through a virtual care platform. 

 

We are looking for an RN to join the eHomecare team as a Directing Registered Nurse (DRN).

The DRN works from their home on a private computer accessing information sent by the eHomeCare Support Worker via a handheld smart phone to a secure portal, a chat line, and phone communication.  The DRN directs the eHomeCare Support Worker to provide safe, compassionate, skill and evidence-based palliative/pediatric care to patients in their own home, ensuring the patient’s symptoms are managed and the family is supported.

 

 This is a Casual remote position 

 

Hours of work are overnights inclusive of weekends.

What The Role Involves

  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Working closely with the eHomeCare Support Workers to provide exceptional care to patients through a virtual means
  • Developing meaningful relationships with eHomeCare Support Workers while mentoring them to increase their overall development in observing, acquiring information and effective reporting
  • Being available throughout the entire shift electronically through the portal, chat line and by phone to the eHomeCare Support Worker, patient, and family as well as other healthcare providers
  • Working with eHomeCare Support Workers in compliance with the CNO document Working with Unregulated Care Providers
  • Performing other clinical duties as required

What You Bring

  • Registered Nurse (RN) degree
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Two years of recent acute care/community-based nursing experience
  • Experience with Pediatrics and palliative care
  • Knowledge in hospice palliative care - its philosophy, holistic approach to care, effective symptom assessment and management abilities
  • A commitment to providing excellent care at end of life
  • Exceptional interpersonal skills
  • Consistent access to highspeed internet
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test 
  • We require a current vulnerable sector check and a clear background check
