PSWs are an integral part of a health care team, and in this role, you will encourage, support, and promote independence for patients in home and community care settings by assisting with activities of daily living under the supervision of our Personal Support Manager.
This role is part of our Intensive Personal Support Services (IPSS) and supports patients coming out of hospital while they are in transition to home care. The patients seen are typically complex and the care plan decreases in frequency as they transition to community PSW care.
We are looking for experienced Personal Support Workers (PSWs) to help us bring patient and family-centered care to in patients in the Oxford County area. Are you someone we should meet?
Hours of work are 0800-2000 (12-hour shifts), 75 guaranteed hours in a bi-weekly rotation.
Must be available to travel thoughout Oxford County.

As the IPSS program supports with high intensity patients, this position requires 2 plus years of PSW or experience, Community experience is preferred.
In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
