These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These CSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).
